New Online Booking Experience

You can book, reschedule, or cancel an appointment using our new online booking tool, Patient Connection. A mobile phone number will be required to receive a multi-factor authentication code to safeguard your health information. You have the option to create a user account for future access or you can continue to access the system by using a different one-time access code each time.

Patient Connection is currently only available to existing Mayfair patients. Existing Mayfair patients will need to create an account to set up a password. The other option for existing patients is to login via SMS. If you are a New Patient to Mayfair and would like to sign up for Patient Connection, please call our contact centre at 1-866-611-2665 for assistance.

Request New Appointments

To request an appointment, you will be prompted to upload a photo of your requisition. Once we receive the requisition and your details, your requisition will be triaged by our team, and you will receive a link to schedule your appointment. You can select the location, date, and time of your appointment.

Changes and Cancellations

Once you log into Patient Connection, you can change or cancel your appointment using the “change” button.

PLEASE NOTE: X-ray exams are walk-in only; an appointment is not required.
