New Online Booking Experience

You can book, reschedule, or cancel an appointment using our new online booking tool, Patient Connection. A mobile phone number will be required to receive a multi-factor authentication code to safeguard your health information. You have the option to create a user account for future access or you can continue to access the system by using a different one-time access code each time.

X-rays do not require an appointment and are done on a walk-in basis. Please see radiology.ca for clinic locations.

Patient Connection is currently only available to existing Mayfair patients. Existing Mayfair patients may create a Patient Connection account or use a one-time login via SMS.

Existing patients cannot use their accounts to submit appointment requests on someone else’s behalf including dependent children. Each patient’s account is specific to that patient. To book an appointment for a dependent, the dependent’s account must be used.

Request New Appointments

To request an appointment, you will be prompted to upload a photo or scan of your requisition. Once we receive the requisition and your details, your requisition will be triaged by our team, and you will receive a link to schedule your appointment. You can select the location, date, and time of your appointment.

Changes and Cancellations

Once you log into Patient Connection, you can change or cancel your appointment using the “change” button.

PLEASE NOTE: X-ray exams are walk-in only; an appointment is not required.
